Submitting Healthcare, Dentalcare and Optional Benefits Claims

Dental claims (Advantage, Comprehensive and Premier Plans)

Great-West is electronically linked with many dentists across Canada. If your dentist has a direct link with us, your claim may be settled right in the dentist's office. The dentist submits the claim electronically and receives confirmation of coverage. You only pay for any expenses not covered by PlanDirect.

To submit a paper claim form:

  1. Complete the Dentalcare Claim Form. [PDF]
  2. Mail the completed form and original receipts to the Individual Health Unit.

Other healthcare claims

Including prescription drugs (for Comprehensive Plan and Basic Plan without the drug card option, and all claims submitted under the Enhanced Prescription Drug benefit), healthcare and visioncare.

For all healthcare expense claims:

  1. Complete the Healthcare Expenses Statement. [PDF]
  2. Mail the completed form and original receipts to the Individual Health Unit.

Hospital Cash claims

If you purchased the Hospital Cash Benefit, you’ll receive your cash benefit automatically when you submit your claim for other hospital expenses. You don’t have to file a separate claim or provide receipts.

Accidental Death and Dismemberment claims

To make a claim for Accidental Death, Dismemberment and Specific Loss benefits, contact your financial security advisor or the Individual Health Unit. They will help you make your claim.

Major Dental Services and Supplies claims

You may submit claims for the Major Dental Services and Supplies benefit in the same way you submit your standard PlanDirect dentalcare claims.

Enhanced Prescription Drug claims

You must use paper claim forms for Enhanced Prescription Drug expenses, even if your plan includes a drug card.

  1. Complete the Healthcare Expenses Statement. [PDF]
  2. Mail the completed form and original receipts to the Individual Health Unit.

Direct Deposit

You can now sign up for direct deposit of your claims payments.
