To make a claim:

Complete the appropriate form below and send it to our Individual Health Unit.

• For medical, vision or drug claims, use the Healthcare Expenses Statement. [PDF]
• For dental claims, use the Dentalcare Claim Form. [PDF]
• For Emergency Travel Medical Benefits claims, use the Out-of-Country Benefits Claim Form.* [PDF]
  *Important: You may also be required to submit a provincial third-party agreement form along with this form.
