What do you want to do?
To get information about your existing coverage

To make changes to your coverage, such as upgrading, downgrading or adding optional benefits
Speak with your financial security advisor, contact PDAdmin Group or complete the Supplementary Application form [PDF] and send it directly to PDAdmin Group.
Your premium will be affected if you make the following changes:
- Add or remove coverage for a spouse or child
- Upgrade or downgrade your Sonata Health coverage
- Add or remove optional coverage
- You, your covered spouse or child moves to another province or territory.
Your premium may change at renewal.
You can upgrade your coverage at any time, for Scales 1 - 4:
- Scale 1 to Scale 2, 3 or 4
- Scale 2 to Scale 3 or 4
- Scale 4 to Scale 3
Guaranteed Acceptance Plan (GAP) – If you chose this plan, you can apply to upgrade your coverage. However, if you were offered GAP based on your medical history, you can’t re-apply or upgrade your coverage.
You’ll have to provide updated medical information when you apply to upgrade your coverage. Any upgrade in coverage must be approved by Great-West, and is based on the medical information you provide.
You can change to a plan with a lower level of coverage if you have had your current plan for at least one year.
You can add optional benefits at any time (Scales 1 to 4 only).
To add or remove Sonata Health coverage for your spouse or children
To add coverage for a spouse (or if your spouse has changed and you want to add coverage for him/her), your spouse must complete the Medical and Lifestyle Questionnaire on the Supplementary Application form [PDF]. Eligibility for coverage is based on your spouse's medical history.
To add coverage for a child:
- On the birth of a child, if you already have family coverage, you can add coverage for the child by notifying PDAdmin Group within 31 days of the birth.
- On the birth of a child, if you do not have family coverage, you can add coverage for the child by simply notifying PDAdmin Group within 31 days of the birth.
- To add coverage for an adopted child or for children of a new spouse, you will have to complete the Medical and Lifestyle Questionnaire on the Supplementary Application form [PDF], on behalf of the child.
To update your personal information, such as your name or address
Contact PDAdmin Group.
To change your method of payment for your Sonata Health premiums
Contact PDAdmin Group.
To change your beneficiary (if your Sonata Health plan includes the accidental death, dismemberment and specific loss benefit)
Complete the Change of Beneficiary form [PDF] and send it to PDAdmin Group.
You will receive written confirmation that your beneficiary has been changed.
If you live in Quebec, the designation of your spouse as beneficiary is irrevocable, unless you specifically make the designation revocable. You will not be able to change an irrevocable beneficiary designation without your spouse’s written consent.
If your beneficiary is a minor, we recommend you appoint a trustee using the form below.

To apply for reconsideration of rating or exclusion
Complete the Application for Reconsideration of Rating or Exclusion form [PDF] and send it to PDAdmin Group.
To renew your Sonata Health coverage
Your policy is for a term of one year, beginning on the effective date shown on your Policy Specifications. You do not have to do anything to renew your Sonata Health policy; your policy will automatically be renewed each year, on your policy's annual renewal date, which is also shown on your Policy Specifications. Your Sonata Health coverage can continue until you reach age 65.
At each renewal, Great-West will re-establish your premium. Your new premium will take effect on the annual renewal date. Each year, your premium will be changed to reflect such things as increasing healthcare costs, shifts in expenses from provincial to private plans, and whether you or your child(ren) have moved into a new age band. This means your premium may increase on renewal. You will be notified at least 31 days before any change in premium.
To cancel your Sonata Health plan, or to cancel coverage for spouse and/or child(ren)
Submit a written request to your financial security advisor or PDAdmin Group.
Your coverage will end on the last day of the month in which you cancel your coverage. If you've paid premium in advance, it will be refunded as of the first of the month following your cancellation.
